Our patient portal is an easy-to-use tool to help ensure we have your correct demographic and medical information before we even see you. This will not only make us more efficient but it will enable us to provide you the best care. If you have any trouble with the portal after reading the following instructions, please call us at (804) 326-4448 for help! We don’t want this process to be intimidating, but rather empowering, as it allows for easy communication between our patients and medical team.

Below is a step-by-step tutorial designed to help you activate and navigate our portal. This picture tutorial will show you how to activate, login, and reset your password, in addition to showing you where to find and update your chart information.

 After your portal is activated by our staff, you will receive an email from “Patient Portal” with a link to the portal. Make sure you are using Mozilla Firefox or Chrome as your browser.

 

 

Click on the link and then “Continue as Patient”. Follow the instructions to create a password.

 

 

If you do not use the link in the initial email within 72 hours, simply click on “Forgot Password” to establish a username and password.

 

 

Once you  have established a username and password, type them in the appropriate fields and click “Log In.”

 

 

If you ever forget your password, click “Forgot Password.”

 

 

When resetting your password, enter your username and birthday. Your birthday needs to be  in the “MM/DD/YY” format. For example, if your birthday is June 1, 1984, enter 06/01/84. Enter your last name, then click the box confirming you are not a robot, then “Request Email.”

 

 

After you have successfully logged into our portal, this will be your homescreen. Use the My Health tab on the top left menu to confirm and update you insurance, pharmacy, medications, allergies, etc.

To review your upcoming and previous appointments, please press ”Appointments” on the top left of the menu. “Upcoming Appointments” and “Past Appointments” tabs are on the left hand side.

 

 

To view, download, and/ or print your visit note for your own records, click on "Past Appointments" on the left hand menu. A list of your past appointments will show up. Select the visit for which you need to see your note.

 

 

A pdf of your visit note will show. In the top right corner of that window, there will be icons to download or print the pdf.
